We have a dedicated Preventative Health Team who conduct health assessments (bulk billed) and provide patients with the support needed to ensure that chronic conditions don’t impact your quality of life, and we help manage your condition to keep you out of hospital.
As you grow older, the risk of medical complications relating to certain health conditions may increase. This may require greater diligence and care from your doctor to properly manage these conditions. At CHMC, we can also organise Team Care Arrangements for you with Allied Health Professionals that assist you in managing your chronic disease.

Castle Hill Medical Centre has a dedicated Preventative Heath Team who conduct health assessments (bulk billed) and can organise Team Care Arrangements for you with Allied Health Professionals that will assist you in managing your Chronic Disease.
The appointment to organise the referral is bulk billed and the Allied Health professional appointments will be subsidised by Medicare. The Preventative Health Team also provide basic Diabetic education and support to patients that are also bulk billed.
