Note: This form cannot be used for direct communication with doctors and is not to be used for booking appointments. All script and referral requests will require a telehealth appointment to be made with your requested Doctor. This will incur a fee of $87.75. You will be entitled to a Medicare rebate of $39.75 if you have seen a doctor face to face in our centre in the last 12 months.
